Primary Responsibilities :
1. Medical & Dialysis-Related Support
Accompany the patient to dialysis sessions three times per week and ensure timely arrival and safe travel to and from the hospital.
Support the patient during dialysis visits, which may last approximately 4 hours per session.
Closely observe the patient's condition before and after dialysis, including fatigue, discomfort, weakness, or any unusual symptoms.
Remain in communication with the designated guardian and provide regular updates regarding the patient's condition and dialysis progress.
Follow instructions provided by the guardian and coordinate appropriately with nurses and medical staff.
Administer medicines strictly according to the prescribed schedule and instructions provided by the family/guardian.
Maintain a daily medication log.
Keep track of prescriptions, medicine refills, medical reports, and upcoming doctor appointments.
Immediately inform the family or guardian in case of any medical concern or emergency.
2. Nutrition & Meal Support
Ensure meals are taken on time according to the patient's daily routine.
Follow dietary instructions provided by the doctor or dietician.
Support adherence to dialysis-related dietary restrictions, including prescribed limitations regarding sodium, potassium, fluids, or other dietary requirements.
Monitor appetite and report significant changes to the family.
Encourage hydration strictly within medically prescribed limits.
3. Daily Personal Care
Assist with bathing, dressing, grooming, and maintaining personal hygiene.
Support mobility, including walking assistance, getting in and out of bed, and wheelchair assistance when required.
Ensure the patient is comfortable and safely positioned during rest and sleep.
Monitor daily routines and general physical comfort.
Maintain the patient's dignity, privacy, and independence wherever possible.
4. Emotional Support & Companionship
Provide genuine companionship, conversation, and emotional reassurance.
Spend quality time with the patient and help prevent feelings of loneliness or isolation.
Be attentive to changes in mood, anxiety, or emotional well-being.
Encourage suitable light activities, conversations, reading, or hobbies based on the patient's health and interests.
Treat the patient with warmth, patience, respect, and compassion.
5. Safety & Household Support
Keep the patient's immediate living space clean, organized, and free from potential hazards.
Ensure a safe environment to minimize the risk of falls or injuries.
Maintain basic daily records related to:
Medicines
Meals
Blood pressure
Blood sugar (if instructed)
Weight
Dialysis reports
General observations
Respond calmly and promptly during emergencies.
Know when to immediately inform family members, contact medical professionals, or seek emergency assistance.
Experience Requirements:
Candidate Requirements
Prior experience in elderly care, patient care, nursing assistance, or personal caregiving.
Experience caring for patients undergoing dialysis, diabetes, or other chronic medical conditions will be preferred.
Basic understanding of medication schedules and patient monitoring.
Ability to monitor basic vitals such as blood pressure, blood sugar, and weight, if required.
Physically fit and capable of assisting with mobility and daily activities.
Strong sense of responsibility, punctuality, and time management.
Good communication skills to coordinate with family members, guardians, nurses, and medical professionals.
Patient, compassionate, trustworthy, and emotionally mature.
Ability to remain calm and attentive during medical or emergency situations.
References from previous caregiving roles will be preferred.
